ABSTRACT

Introducción: Uno de los aspectos que más interés suscita en lo referente a la distribución geográfica de la mortalidad por cáncer es la formación de conglomerados espaciales. Objetivo: Identificar el patrón espacial de la mortalidad por cáncer, cardiopatía isquémica y enfermedad cerebrovascular isquémica mediante la detección y descripción de conglomerados espaciales en la provincia de Santiago de Cuba. Métodos: Se realizó un estudio ecológico exploratorio espacial de los fallecimientos por cáncer, cardiopatía isquémica y enfermedad cerebrovascular isquémica durante el período comprendido desde el 1 de enero hasta el 31 de diciembre de 2019 en la provincia de Santiago de Cuba, para lo cual se procedió a la detección de conglomerados espaciales de elevada y baja mortalidad por las causas antes citadas. Se empleó el método de escaneo espacial estadístico con el programa Satscan y las variables independientes fueron divididas en demográficas y clínicas. Resultados: De los 36 conglomerados espaciales de mortalidad detectados, 23 correspondieron al cáncer (65,8 %), 7 a cardiopatía isquémica (18,4 %) y 6 a enfermedad cerebrovascular isquémica (15,8 %); asimismo, 44,7 % del total eran de elevado riesgo y 55,3 % de bajo riesgo. Conclusiones: El patrón de la distribución espacial de mortalidad por cáncer de próstata, pulmón, mama, colon, esófago, cardiopatía isquémica y enfermedad cerebrovascular isquémica se caracterizó por la formación de conglomerados espaciales de elevada y baja mortalidad.


Introduction: One of the aspects that more interest raises regarding the geographical distribution of mortality due to cancer is the formation of space conglomerates. Objective: To identify the space pattern of mortality due to cancer, ischemic heart disease and ischemic cerebrovascular disease by means of the detection and description of space conglomerates in Santiago de Cuba province. Methods: A space exploratory ecological study of deaths due to cancer, ischemic heart disease and ischemic cerebrovascular disease was carried out during January 1st to December 31, 2019 in Santiago de Cuba province, for which space conglomerates of high and low mortality due to the abovementioned causes were detected. The statistical space escanning method was used with the Satscan program and the independent variables were divided in demographic and clinical. Results: Of the 36 space conglomerates of mortality detected, twenty three corresponded to cancer (65.8%), seven to ischemic heart disease (18.4%) and six to ischemic cerebrovascular disease (15.8%); also, 44.7% of the total was of high risk and 55.3% of low risk. Conclusions: The space distribution pattern of mortality due to prostate, lung, breast, colon, esophagus cancer, ischemic heart disease and ischemic cerebrovascular disease was characterized by the formation of high and low mortality space conglomerates.

Introducción: La enfermedad cerebrovascular (ECV) es una urgencia neurológica que en ocasiones se encuentra influenciada por los cambios del tiempo. La ECV isquémica es una emergencia médica con una ventana estrecha para su diagnóstico y tratamiento. El daño generado por esta enfermedad se estima mediante de la mortalidad, discapacidad e impacto social. Objetivo: Proponer un programa de capacitación basado en los efectos fisiopatológicos que provocan en los pacientes, cambios de tiempo y ritmo circadiano en el contexto de la adaptación al cambio climático. Método: Se desarrolló un estudio multietápico prospectivo en el Hospital Universitario Mártires del 9 de Abril del municipio Sagua la Grande entre los años 1993 a 2017. En la primera etapa se seleccionó un universo de 52 profesionales de la salud que atendieron a estos pacientes; en la segunda, 48 y en la tercera, 61. Resultados: El nivel de conocimiento del personal de salud en una primera etapa era inadecuado (78,85 %); en la tercera, este porcentaje disminuyó (14,75 %). Como resultado de la capacitación mejoraron los tiempos de atención a los pacientes con ECV a su llegada al hospital, y en la tercera, más del 50 % de los enfermos fueron atendidos antes de las seis horas de inicio de los síntomas. Conclusiones: La capacitación sobre ECV basada en los efectos fisiopatológicos que provocan en los pacientes el cambio climático y el ritmo circadiano disminuyó el tiempo de espera, y se asoció con una reducción de la morbilidad y la mortalidad, después de aplicar este programa.


Introduction: cerebrovascular disease (CVD) is a neurological emergency that is sometimes influenced by climate changes. Ischemic CVD is a medical emergency with a narrow window for diagnosis and treatment. The damage generated by this disease is estimated through mortality, disability and social impact. Objective: to propose a training program based on the pathophysiological effects in patients caused by changes in time and circadian rhythm in the context of adaptation to climate change. Methods: a multistage prospective study was conducted at "Mártires del 9 de Abril" University Hospital in Sagua la Grande municipality between 1993 and 2017. A universe of health professionals who cared for these patients was selected as follows: 52 in the first stage; 48 in the second stage, and 61 in the third ones. Results: the level of knowledge of health personnel in the first stage was inadequate (78.85%); this percentage decreased in the third stage (14.75%). Care times for CVD patients improved upon arrival at the hospital as a result of the training, as well as in the third ones, more than 50% of the patients were seen within six hours of the onset of symptoms. Conclusions: CVD training based on the pathophysiological effects of climate change and circadian rhythm on patients decreased waiting time, and was associated with reduced morbidity and mortality, after applying this program.

ABSTRACT

Objective: To identify the worldwide bibliometric characteristics of research on SARS?CoV?2 infection and cerebrovascular disease. Methods: A retrospective, descriptive, and bibliometric study was performed. We analyzed 1834 publications about COVID?19 and cerebrovascular disease from the Scopus database considering the time since the beginning of the pandemic between 2019 and 2020. Bibliometric indicators were evaluated such as number of citations, citations per publication by authors, countries, journals, and collaborations at national, international, institutional, and impact levels according to Cite Score Quartile and h?index metrics. All analysis was performed using SciVal software. Results: The highest percentage of articles corresponded to universities in the United States, including Harvard and New York with 59 and 20 publications, respectively, and the University of Toronto in Canada with 22 publications. In relation to citation indicators, journals such as Stroke and Journal Stroke and Cerebrovascular diseases obtained 1971 and 561 citations, respectively. Regarding collaboration indicators, the national collaboration index was 39.4% and the institutional collaboration index was 31.1%. Finally, neurology, cardiovascular medicine, and cardiology and surgery were the subject areas with the highest research results, with 424, 217, and 128 studies, respectively. Conclusion: It was observed that the United States was the country with the highest scientific production on COVID?19 and cerebrovascular disease in the year 2020 in the different health areas; however, more research is still needed worldwide for a better analysis of the bibliometric indicators on the subject.

ABSTRACT

ObjectiveTo explore the synergetic effect of temperature and PM2.5 on cardiovascular and cerebrovascular diseases. MethodWe collected cardiovascular and cerebrovascular death cases,air pollution and meteorological data during the same period in Pudong New Area from 2013 to 2018.We used generalized additive models (GAMs) with poisson regression including non-stratification model, nonparametric bivariate response model and pollution-stratified parametric model, to assess the interaction between temperature and PM2.5 and on the number of cardiovascular cerebrovascular and cerebrovascular disease deaths. ResultsThe exposure-response relationship between temperature and the number of cardiovascular and cerebrovascular deaths exhibited "U" type and the most comfortable temperature was 18.9 ℃. When the concentrations of PM2.5 increased by 10 μg·m-3, the deaths of total, male, female, ≤75 years and >75 years increased, respectively, by 0.60%(95%CI: 0.30%‒0.91%), 0.77%(95%CI:0.34%‒1.20%), 0.46%(95%CI:0.05%‒0.86%), 0.66%(95%CI:0.03%‒1.30%) and 0.59%(95%CI:0.26%‒0.92%). With the increase of PM2.5 concentration level, the impact of temperature on cardiovascular and cerebrovascular diseases gradually increased, and the impact was the most significant when the concentration of PM2.5 was more than 150 µg·m-3. There were different sensitive people in different seasons. ConclusionPM2.5 concentration levels of mild pollution and above can exacerbate the negative effects of temperature on cardiovascular and cerebrovascular diseases.

ABSTRACT

This paper elaborated a case of an elderly patient with anti-contactin-associated protein-like 2 (CASPR2) antibody-associated autoimmune encephalitis. It illustrated the common manifestations and diagnostic process of anti-CASPR2 antibody-associated encephalitis, and the analysis of clinical symptoms and ancillary findings had contributed to gaining insight into the diagnosis of autoimmune encephalitis in elderly patients characterized by distinct psycho-behavioral abnormalities and cognitive decline, as well as suggesting the possibility that autoimmune encephalitis in elderly may exacerbate the development of cerebrovascular disease.

ABSTRACT

This study intended to evaluate the efficacy and safety of single Hirudo prescriptions in the treatment of ischemic cerebrovascular disease(ICVD) by frequency network Meta-analysis and traditional Meta-analysis. CNKI, Wanfang, VIP, SinoMed, PubMed, EMbase, and Cochrane Library databases were searched to collect the randomized controlled trial(RCT) of single Hirudo prescriptions for ICVD from the inception of the databases to May 2022. The quality of the included literature was evaluated by Cochrane risk of bias tool. Finally, 54 RCTs and 3 single Hirudo prescriptions were included. Statistical analysis was conducted by RevMan 5.3 and Stata SE 15. Network Meta-analysis showed that in terms of the clinical effective rate, the surface under the cumulative ranking curve(SUCRA) of intervention measures was as follows: Huoxue Tongmai Capsules+conventional treatment>Maixuekang Capsules+conventional treatment>Naoxuekang Capsules+conventional treatment>conventional treatment. Traditional Meta-analysis revealed that in terms of the safety of ICVD treatment, Maixuekang Capsules+conventional treatment had higher safety than conventional treatment alone. According to the network Meta-analysis and traditional Meta-analysis, it was found that conventional treatment combined with single Hirudo prescriptions improved the clinical efficacy of ICVD patients, and compared with that of conventional treatment alone, the incidence of adverse reactions of combined treatment was low and the safety was high. However, the methodological quality of the articles included in this study was generally low and there were large differences in the number of articles on the three combined medication. Therefore, the conclusion of this study needed to be confirmed by subsequent RCT.

Introducción: El accidente cerebrovascular (ACV) es una afección con alta morbimortalidad, producida por la interrupción de la perfusión cerebral. Este artículo tiene como objetivo analizar la tendencia de egresos hospitalarios por ACV agudo en el periodo 2018-2021 en Chile. Metodología: Estudio descriptivo observacional, que incluyó egresos hospitalarios con diagnóstico de ACV agudo en el periodo 2018-2021, en Chile (n=19.274), según datos del Departamento de Estadísticas e Información de Salud de Chile. Se calculó la tasa de egreso hospitalario (TEH) según variable, utilizando datos del censo chileno 2017. No requirió comité de ética. Resultados: El 2018 tuvo la mayor TEH por ACV agudo (28,99) y el 2021 la menor (26,39). El grupo de "80 años y más" presenta la mayor TEH, mientras que el de "5-9 años" la menor. La mayor y menor TEH las tienen las regiones de Ñuble (263,00) y Tarapacá (10,29), respectivamente. Discusión: La disminución de TEH durante dicho periodo podría deberse al fortalecimiento de la Ley de Urgencia, al Programa Nacional de Telesalud y el impacto de la reciente pandemia. A mayor edad, aumenta la prevalencia y gravedad de las comorbilidades, lo cual explicaría la mayor TEH en el grupo más longevo. La mayor TEH masculina podría ser porque los hombres presentan mayor cantidad y severidad de factores de riesgo. La mayor TEH en la región del Ñuble posiblemente sea por los elevados niveles de pobreza y ruralidad, y la menor TEH en Tarapacá podría relacionarse con la presencia de una población nacional más joven.


Introduction: Stroke (CVA) is a condition with high morbidity and mortality, produced by the interruption of cerebral perfusion. This article aims to analyze the trend of hospital discharges for acute stroke in the period 2018-2021 in Chile. Methodology: Descriptive observational study, which included hospital discharges with a diagnosis of acute stroke in the period 2018-2021, in Chile (n=19,274), according to data from the Chilean Department of Health Statistics and Information. The hospital discharge rate (HTE) was calculated according to variable, using data from the 2017 Chilean census. No ethics committee was required. Results: 2018 had the highest HTE for acute stroke (28.99) and 2021 the lowest (26.39). The group aged "80 years and older" had the highest TEH, while the group aged "5-9 years" had the lowest TEH. The regions of Ñuble (263.00) and Tarapacá (10.29) have the highest and lowest HTE, respectively. Discussion: The decrease in HTE during this period could be due to the strengthening of the emergency law, the National Telehealth Program and the impact of the recent pandemic. The higher the age, the higher the prevalence and severity of comorbidities, which would explain the higher HTE in the older group. The higher male HTE may be due to the fact that men have a greater number and severity of risk factors. The higher HTE in the Ñuble region is possibly due to the high levels of poverty and rurality, and the lower HTE in Tarapacá would be related to having a younger national population.

Abstract Background Atherosclerosis is a condition in which fats, cholesterol, fibrin, and other substances accumulate into plaque on the arterial walls. Plaque can harden and narrow the arteries, in turn limiting the blood flow and resulting in diseases, such as acute myocardial infarction (AMI), ischemic stroke (IS), or peripheral arterial disease (PAD). There is a fairly high risk of a secondary atherosclerotic event if patients are not treated after the primary episode. Objective To calculate the statistical probability of developing AMI, IS, or PAD after treating the primary disease. Methods Data for statistical probability studies included 507,690 patients with primary atherosclerotic disease, who were in treatment during the study period and who did or did not develop a secondary atherosclerotic disease event. Result Statistical probability data indicate that few AMI patients can develop IS (2.99%) or PAD (2.86%) as a secondary disease. Patients with primary diagnoses of IS showed a 5.07% risk of developing PAD and a 0.95% risk of developing AMI; however, PAD patients showed a higher probability for both AMI (9.17%) and IS (8.79%). Conclusion Secondary atherosclerotic disease episodes after IS, AMI, and PAD were confirmed by statistical probability and are consistent with data from the literature. The study revealed that a primary PAD event leads to high rates of secondary episodes, and special attention should be given to the diagnosis and treatment of PAD in order to decrease the occurrence of secondary events.

Objective:To explore the characteristics of the association between the triglyceride glucose (TyG) index and nonfatal cardio-cerebrovascular disease risk in a community population.Method:This was a prospective cohort study. From December 2011 to April 2012, the first investigation was conducted among subjects with more than 40-year old who were from Shijingshan district and Pingguoyuan community in Beijing. The second investigation was conducted from April to October 2015. All the subjects were divided into three groups according to the tertile of the TyG index at baseline. The multivariate Cox proportional risk regression model was established to explore the correlation between the TyG index and nonfatal cardio-cerebrovascular disease risk and the Kaplan-Meier survival curve of the TyG index group was drawn. Subgroup analyses were performed according to age, gender, body mass index, type 2 diabetes mellitus (T2DM), hypertension, and hyperlipidemia to determine the correlation characteristics between the TyG index and nonfatal cardio-cerebrovascular disease among subgroups.Results:A total of 9 577 subjects were finally included to analyze. The mean follow-up time of this study was (34.14±3.84) months. During the follow-up, 363 subjects (3.8%) occurred nonfatal cardio-cerebrovascular disease. The multivariate Cox regression analysis results showed that the hazard ratio ( HR) of nonfatal cardio-cerebrovascular disease in the high TyG index group was 1.54 (95% CI 1.19-1.98), 1.60 (95% CI 1.23-2.10), and 1.57 (95% CI 1.20-2.05) in the three models, compared with the low TyG index group. The Kaplan-Meier analysis showed that the risk of nonfatal cardio-cerebrovascular disease increased from the low-TyG index group to the high-TyG index group ( P=0.015). In the six subgroups analysis, only gender was shown to have a significant interaction effect with the TyG index and nonfatal cardio-cerebrovascular disease risk. In the female population, the risk of nonfatal cardio-cerebrovascular disease is significantly increased with the increase in the TyG index level ( P<0.001). Conclusions:A high TyG index is independently related to the increased risk of nonfatal cardio-cerebrovascular disease in the Beijing community population. Gender has a significant interaction with the TyG index and nonfatal cardio-cerebrovascular disease risk. Therefore, the TyG index may be a useful marker to predict the nonfatal cardio-cerebrovascular disease risk of a community population.

Objective:To analyze the disease characteristics and hospitalization burden of elderly inpatients with cerebrovascular disease, so as to provide basis for disease prevention, diagnosis, treatment and rehabilitation of elderly patients with multiple chronic conditions.Methods:The data of the first page of medical records of elderly inpatients with cerebrovascular disease from 2015 to 2020 in a comprehensive tertiary hospital in Jiangsu Province were retrospectively collected.Descriptive analysis, variance analysis and multiple linear regression analysis were carried out for the research objects using SPSS statistics and Python complex network methods.Results:A total of 14 657 elderly inpatients with cerebrovascular disease were included.From 2015 to 2020, the number of hospitalizations increased from 1 268 to 4 733, the average number of hospitalizations increased from 2.0 to 2.9, the average length of stay decreased from 11.9 days to 9.1 days, and the average number of illnesses increased from 1.9 to 4.9.The five most common comorbidities associated with cerebrovascular diseases were hypertension, diabetes, ischemic heart disease, other types of heart disease and other respiratory diseases.From 2015 to 2020, the average hospitalization costs of elderly cerebrovascular inpatients decreased from 20588.1 Chinese yuan(CYN)to 15580.9 Chinese yuan(CYN). The hospitalization cost was mainly composed of drug cost(46.6%), diagnosis cost(28.2%)and treatment cost(20.2%). There were significant differences in the average hospitalization expenses among patients with different gender, age, number of admissions, length of stay and number of diseases( P<0.05 for all). Gender, age, number of hospitalizations and length of stay had an impact on hospitalization expenses. Conclusions:The number of inpatients, comorbidities, and hospitalized times of elderly patients with cerebrovascular disease showed an increasing trend, while the number of hospitalization days and the average hospitalization cost showed a downward trend.Comorbidities associated with cerebrovascular diseases should be one of the concerns of chronic disease management in the elderly.

Introducción: El 80 por ciento de los accidentes cerebrovasculares resulta de tipo isquémico y al menos el 20-30 por ciento está provocado por estenosis de la arteria carótida extracraneal. La endarterectomía carotídea tiene como principal objetivo eliminar las placas de ateroma, que son fuentes de émbolos para el encéfalo, y mejorar el flujo sanguíneo a este órgano. Objetivo: Caracterizar la morbilidad de la endarterectomía carotídea en el Hospital General Docente "Enrique Cabrera" desde 2018 hasta 2021. Métodos: Se realizó un estudio de tipo descriptivo y corte transversal a 32 pacientes, a los cuales se les realizó endarterectomía carotídea. La recolección de los datos se efectuó mediante la revisión de historias clínicas. El período de estudio abarcó desde enero de 2018 hasta enero de 2021. Resultados: Predominó el sexo masculino (65,6 por ciento) y la media de edad fue de 68,8 años. Los factores de riesgo más frecuentes resultaron la hipertensión arterial y el tabaquismo con 75 por ciento y 65,6 por ciento, respectivamente. La enfermedad cerebrovascular con secuela mínima constituyó la forma clínica más frecuente en el 40,6 por ciento de los casos. Se encontró asociación estadísticamente significativa entre diabetes mellitus y formas clínicas, y entre enfermedad arterial periférica y la aparición de complicaciones. La carótida izquierda resultó la más afectada con 53 por ciento y el porcentaje de estenosis, entre 70 por ciento y 99 por ciento, se encontró en el 55,9 por ciento de los pacientes. Con mayor frecuencia se presentaron las complicaciones: lesión de nervios craneales (5,8 por ciento) y hematoma cervical (5,8 por ciento). La endarterectomía por eversión se realizó en el 97 por ciento de los casos. Conclusiones: Se demostró la seguridad y el éxito de la endarterectomía carotídea. La mayoría de los pacientes tuvo una evolución satisfactoria, y se presentaron escasas complicaciones y mortalidad nula(AU)


Introduction: 80 percent of strokes result in ischemic type and at least 20-30 percent are caused by stenosis of the extracranial carotid artery. Carotid endarterectomy has as its main objective to remove atheromatous plaques, which are sources of emboli for the brain, and improve blood flow to this organ. Objective: To characterize the morbidity of carotid endarterectomy at the "Enrique Cabrera" General Teaching Hospital from 2018 to 2021. Methods: A descriptive, cross-sectional study was conducted in 32 patients, who underwent carotid endarterectomy. Data collection was carried out by reviewing medical records. The study period spanned from January 2018 to January 2021. Results: The male sex predominated (65.6 percent) and the mean age was 68.8 years. The most frequent risk factors were high blood pressure and smoking with 75 percent and 65.6 percent, respectively. Cerebrovascular disease with minimal sequelae was the most frequent clinical form in 40.6 percent of cases. A statistically significant association was found between diabetes mellitus and clinical forms, and between peripheral arterial disease and the occurrence of complications. The left carotid was the most affected with 53 percent and the percentage of stenosis, between 70 percent and 99 percent, was found in 55.9 percent of patients. The following complications were the most common: cranial nerve injury (5.8 percent) and cervical hematoma (5.8 percent). Eversion endarterectomy was performed in 97 percent of cases. Conclusions: The safety and success of carotid endarterectomy were demonstrated. Most of the patients had a satisfactory evolution, and there were few complications and zero mortality(AU)

ABSTRACT

Introducción: la enfermedad vascular cerebral (EVC) es un déficit neurológico súbito causado por alteraciones en la circulación cerebral; considerada por la Organización Mundial de la Salud (OMS) como la segunda causa global de muerte en el mundo, en el 2020 ocupó el séptimo lugar como causa de muerte, en México es un problema de salud pública y una importante causa de discapacidad. Objetivo: realizar un estudio de caso a una persona con EVC hemorrágico a través del proceso de atención de enfermería. Método: plan de cuidados con el modelo de Virginia Henderson; fuentes de información: directa, hoja de enfermería y expediente clínico. Se graficaron signos vitales y presión intracraneal PIC. Análisis de artículos vigentes en PubMed, Redalyc, SciELO, Elsevier. Descripción del caso: masculino hipertenso en descontrol y fumador moderado. En el servicio de urgencias presenta datos de deterioro rostro caudal en fase bulbar, se da manejo avanzado de la vía aérea e ingresa a quirófano para colocación de ventriculostomía. Con probable mortalidad del 97 %. Consideraciones éticas: principios éticos para la investigación en la Escuela Nacional de Enfermería y Obstetricia, ENEO, Código Deontológico de Enfermería, Código de Ética para las Enfermeras y Enfermeros de México y NOM 004 del expediente clínico. Conclusiones: se emplearon cuidados especializados a necesidades alteradas según modelo de Henderson que continúa siendo actual como filosofía adaptativa para valoración integral del ente de nuestros cuidados. Mejoró mi curva de aprendizaje en conocimiento sensible e intelectual con enfoque crítico y neurológico acorde a la enfermería basada en la evidencia. Las EVC son causa de muerte y discapacidad, no deben ser subestimadas sino objeto de atención de instituciones gubernamentales y de salud a nivel mundial pues falta generar cultura de prevención.


Introduction: CVD is a sudden neurological deficit caused by alterations in cerebral circulation; considered by the WHO as the second global cause of death in the world, in 2020 it ranked seventh as a cause of death in Mexico and an important cause of disability. Objective: to carry out a case study of a person with Hemorrhagic CVD through the Nursing Care Process. Method: care plan with the Henderson model; Information sources: direct, Nursing Sheet and Clinical file. Vital signs and ICP were graphed. Analysis of current articles in PubMed, Redalyc, SciELO, ELSEVIER. Case description: patient is uncontrolled hypertensive and a moderate smoker. In the emergency department, he presented data of facial caudal deterioration in the bulbar phase, advanced management of the airway was given, and he entered the operating room for ventriculostomy placement. With Mortality of 97%. Ethical considerations: ethical principles for research in the ENEO, Nursing Code of Ethics, Code of ethics for nurses in Mexico and NOM 004 of the clinical file. Conclusions: specialized care was used for altered needs according to the Henderson model, which continues to be current as an adaptive philosophy for comprehensive assessment of the entity of our care. It improved my learning curve in sensitive and intellectual knowledge with a critical and neurological approach according to evidence-based nursing. CVDs are a cause of death and disability, they should not be underestimated, but rather the object of attention from governmental and health institutions worldwide since it is necessary to generate a culture of prevention.

ABSTRACT

Abstract This study aimed at estimating the prevalence of stroke in older adults in Brazil, and at identifying the sociodemographic, health-related, health service-related, and environmental factors associated with independence in daily activities. Across-sectional, population-based study (Brazilian Longitudinal Study of Aging 2015-2016) was conducted. 536 individuals (≥ 50 years), from 9,412 participants, have had stroke and were included. Prevalence of stroke was 5.3% among individuals aged 50 years and over, increasing up to 8.0% among individuals aged 75 years and over, showing a dissimilar pattern between sex. Independence was associated with walking speed (Prevalence Ratio (PR) 2.72, 95%CI: 1.96 to 3.77), physical activity (PR 1.24; 95%CI: 1.04 to 1.47) and use of walking devices (PR 0.63; 95%CI: 0.41 to 0.96). A significant interaction was found between walking speed plus home adaptations and performance of daily living activities (PR 3.42; 95%CI: 1.04 to 11.29). The probability of independence was 40% among slow walkers (< 0.4 m/s), increasing up to 70% among fast walkers (> 0.8 m/s), and to 90% among those who also have home adaptations. Faster walking speed combined with home adaptations was the main factor associated with long-term independence after stroke.


Resumo Objetivou-se estimar a prevalência do acidente vascular cerebral (AVC) em brasileiros mais velhos, bem como identificar fatores sociodemográficos, de saúde, de serviços de saúde e ambientais associados à independência em atividades de vida diária. Trata-se de um estudo transversal de base populacional (Estudo Longitudinal da Saúde de Idosos Brasileiros 2015-2016). Dos 9.412 participantes, 536 indivíduos (≥ 50 anos) tiveram AVC e foram incluídos. A prevalência do AVC foi de 5,3% em indivíduos com 50 anos e mais, aumentando para 8% entre indivíduos com 75 anos e mais, com padrão dissimilar entre sexo. A independência foi associada à velocidade da marcha (razão de prevalência [RP] 2,72, IC95%: 1,96-3,77), atividade física (PR 1,24; IC95%: 1,04-1,47) e uso de dispositivos auxiliares de marcha (RP 0,63; IC95%: 0,41-0,96). Encontrou-se interação significativa para velocidade da marcha, adaptações no domicílio e a independência (RP 3,42; IC95%: 1.04-11.29). A probabilidade de independência foi de 40% nas velocidades mais baixas (< 0,4 m/s), e nas velocidades mais elevadas (> 0,8 m/s) foi de 70%, aumentando para 90% quando há adaptações no domicílio. A velocidade rápida da marcha e a adaptação no domicílio foram os principais fatores associados à independência a longo prazo após o AVC.

ABSTRACT

Tuberculous meningitis (TBM) is an air-borne infectious disease caused by the bacteria Mycobacterium tuberculosis that affects the central nervous system (CNS). Among all the incident cases of TB, CNS TB represents approximately 1% with TBM as the most grievous among all. The basic pathology in TBM is the inflammation of the arachnoid membrane, the pia mater and the cerebrospinal fluid (CSF). It typically presents as mild fever, headache, anorexia and general debility that progress over one to two weeks to cause severe headache, fever, vomiting, confusion, meningismus and cranial nerve deficits. The most common complications of TBM include hydrocephalus, optico-chiasmatic arachnoiditis, seizures and stroke. Out of these, tuberculous cerebrovascular disease is a common neurological sequelae. This case study elaborates the treatment line and observations made in a 29 year old male patient who presented with hemiparesis and significant sensory deficit following an event of tuberculous meningitis. MRI brain was suggestive of basilar meningitis, optico-chiasmatic arachnoiditis, infarcts and tuberculoma with chest X-ray revealing increased bronchovascular markings in bilateral lung fields. Initially on admission, Deepana-pachana was done followed by snehapana with Shadpala ghrta and Virechana as Sodhana karma. Abhyanga, Ushma Sweda, Churna pinda sweda and Jambeera pinda sweda were successively done allied with physiotherapy. Yogavasti with Vedanasthapana gana as Kashaya and Kalka was done intervened by Anuvasana vasti. Succeedingly, Murdhni taila prayoga and Marsha nasya were also incorporated with periodical neurological, hematological and biochemical assessment. On discharge, Brahmi kalyanaka ghrta and Brahma Rasayana were advised inclusive of physiotherapy.

RESUMEN Introducción: las enfermedades cardiovasculares son la principal causa de morbilidad y mortalidad, a pesar de las mejoras en los resultados clínicos y la introducción de nuevos fármacos. Objetivo: evaluar la influencia de factores riesgo en el desarrollo de enfermedades cardiovasculares en el adulto de 18 años o más. Método: se realizó un estudio retrospectivo de casos y testigos en pacientes con el diagnóstico de enfermedad cardiovascular que acuden al servicio de medicina interna del Hospital General Universitario "Carlos Manuel de Céspedes", Bayamo, Granma, desde el 5 de enero de 2017 hasta el 31 de diciembre de 2018. Dentro de los factores de riesgo se tuvo en cuenta la edad, género, comorbilidad y los marcadores biológicos. Resultados: dentro de las variables cualitativas el hábito de fumar fue el factor más importante (OR: 3,60; IC: 1,664-7,787; p: 0,001) mientras que de las cuantitativas fue la hipertensión sistólica (OR: 11,50; IC: 1,457-90,896; p: 0,004). El análisis multivariado, mostró que la hipertensión sistólica fue el factor de riesgo independiente más relevante (OR: 11,52; IC del 95%: 3,077- 43,168; p: 0,000) seguido tener como antecedentes de diabetes mellitus (OR: 5,91; IC del 95%: 1,674-20,910). Conclusiones: sobresale como factor de riesgo independiente más importante la hipertensión sistólica, seguida de padecer de diabetes mellitus y tener edad igual o superior a 60 años.


ABSTRACT Introduction: cardiovascular diseases are the main cause of morbidity and mortality, despite improvements in clinical results and the introduction of new drugs. Objective: to evaluate the influence of risk factors in the development of cardiovascular diseases in adults aged 18 years or older. Method: a retrospective study of cases and controls was carried out in patients diagnosed with cardiovascular disease who attend the internal medicine service of the General University Hospital "Carlos Manuel de Céspedes," Bayamo, Granma, from January 5, 2017 to December 31, 2018. Within the risk factors, age, gender, comorbidity and biological markers were taken into account. Results: among the qualitative variables, smoking was the most important factor (OR: 3.60; CI: 1.664-7.787; p: 0.001) while among the quantitative variables it was systolic hypertension (OR: 11.50; CI: 1457-90896, p: 0.004). Multivariate analysis showed that systolic hypertension was the most relevant independent risk factor (OR: 11.52; 95% CI: 3.077-43.168; p: 0.000) followed by having a history of diabetes mellitus (OR: 5.91 ;95% CI: 1674-20910). Conclusions: systolic hypertension stands out as the most important independent risk factor, followed by suffering from diabetes mellitus and being 60 years of age or older.

Objetivo: avaliar a influência dos fatores de risco no desenvolvimento de doenças cardiovasculares em adultos com 18 anos ou mais. Método: foi realizado um estudo retrospectivo de casos e controles em pacientes diagnosticados com doenças cardiovasculares que frequentam o serviço de medicina interna do Hospital Geral Universitário "Carlos Manuel de Céspedes", Bayamo, Granma, de 5 de janeiro de 2017 a 31 de dezembro de 2018 Dentro dos fatores de risco foram considerados idade, sexo, comorbidade e marcadores biológicos. ObjectiveTo analyze the changing trend of cerebrovascular disease burden in Minhang District of Shanghai from 1996 to 2021, and to provide scientific evidence for government to formulate targeted cerebrovascular disease prevention and control strategies. MethodsMortality, years of life lost(YLL), years of lived with disability(YLD) and disability⁃adjusted life years(DALY) were used to evaluate the burden of cerebrovascular diseases in Minhang District. Joinpoint linear regression was used to analyze the trend of disease burden. ResultsFrom 1996 to 2021, the YLL rate of cerebrovascular diseases in Minhang District showed a downward trend (whole population: APC=-1.69%, t=-6.9, P<0.05), The YLD rate of cerebrovascular diseases showed a slow upward trend (whole population: APC=1.17%, t=3.5, P<0.05), The DALY rate of cerebrovascular diseases showed a downward trend, and fluctuated since 2003 (whole population: APC= -1.43%, t=-5.6, P<0.05). The YLL rate of cerebrovascular diseases in men was higher than that in women, and the YLD rate of cerebrovascular diseases in women was higher than that in men. After 2014, the DALY of cerebrovascular diseases in men was higher than that in women. With the increase of age, the burden of cerebrovascular diseases increased, and the burden of disease increased significantly in the age group above 70. ConclusionThe burden of cerebrovascular diseases in Minhang District is at a high level, and there are differences in age, gender and other aspects. Measures such as screening, intervention and rehabilitation need to be improved to reduce disability and premature death caused by cerebrovascular diseases and to reduce the burden of cerebrovascular diseases on individuals, families and society.

ABSTRACT

Abstract Cerebrovascular disease is the second most serious disease in the world. It has the features of high morbidity, high mortality and recurrence rate. Numerous research on the compatibility of Chinese medicine with effective ingredients of cerebral ischemia has been made during the past decades. The purpose of this study is to quantitatively analyze the combined pharmacological effect of effective ingredients in Danshen and Honghua (Dan Hong) on rat microvascular endothelial cells after gradually oxygen-glucose deprivation. The experimental concentration range for the compatibility of two effective ingredients were determined in the preliminary experiments by Cell Counting kit-8 (CCK-8) method. Drugs were added to rat brain microvascular endothelial cells at a non-toxic dose level. After that, the cells were cultured for 12 h, and placed in a hypoxic environment. Finally, the cell survival rate was used as a measure of drug effect. In order to determine synergism or antagonism, the combination index (CI)-isobologram method was performed to analyze the data from the experiments. Based on this theory, the potencies of each drug and the shapes of their does-effect curves are both taken into account. The results show that the synergism or the antagonism between two effective ingredients compatibility change with different proportion and dosage. Furthermore, it can be seen from the results of these experiments that when these drugs are used in combination, the dosage required to achieve the same therapeutic effects is greatly reduced compared with the case of single one. It is worth mentioning that our experiments also prove that the median-effect equation and the CI method can be applied in the field of traditional Chinese medicine.

ABSTRACT Objective To analyze data from children who were previously healthy and presented with post-varicella arterial ischemic stroke upon arrival when admitted to the emergency room, with focus on the clinical/laboratory aspects, and neurocognitive performance after four-year follow-up. Methods Seven children presenting with arterial ischemic stroke after varicella were evaluated at pediatric emergency services in the city of São Paulo (SP), Brazil. Ischemic stroke was determined by magnetic resonance imaging/magnetic resonance angiography in a topography compatible with the areas supplied by the middle cerebral or internal carotid arteries. IgG-class antibodies against varicella zoster virus and varicella-zoster virus DNA by polymerase chain reaction in cerebrospinal fluid were tested. Patients with prothrombotic conditions were excluded. The Pediatric Stroke Outcome Measure was applied upon admission and 4-years after the stroke. Results All patients (age range: 1.3 to 4 years) included presented chickenpox 5.1 (±3.5) months before. All patients had analysis of anti-varicella-zoster-virus-IgG in cerebrospinal fluid, but only three (43%) had a positive result. Of the patients 43% had no vascular lesions identified in magnetic resonance angiography. All patients showed improvement in their sequela scores. After 4 years, five patients displayed good evolution in the Pediatric Stroke Outcome Measure, and only one patient presented with a score of 2 in the sensorimotor and cognition areas. No recurrence of arterial ischemic stroke was observed. Conclusion We reinforced the non-progressive course of post-varicella arterial ischemic stroke after 4-year follow-up. The presence of varicella-zoster-virus-DNA detected by polymerase chain reaction, and/or intrathecal IgG antibody against varicella zoster virus, and angiopathy location in magnetic resonance angiography were not determining for the diagnosis. Invasive tests, with low sensitivity, should be well considered in the diagnosis of post-varicella arterial ischemic stroke.

Objective@#To examine the correlation between atmospheric PM2.5 and emergency call for respiratory diseases.@*Methods@#The daily emergency call for respiratory and cardio-cerebrovascular diseases was collected from Hangzhou Emergency Medical Center from 2018 to 2020, and meteorological and atmospheric pollutant data were collected from Hangzhou Municipal Center for Ecological and Environmental Monitoring during the same period, including daily mean air temperature, daily mean relative humidity, PM2.5, PM10 and SO2 levels. The correlation between atmospheric PM2.5 and emergency call for respiratory and cardio-cerebrovascular diseases was examined using a generalized additive model, and the risk of emergency call was predicted using excessive risk (ER) and its 95%CI.@*Results@#The daily mean emergency call was 14 (interquartile range, 12) cases for respiratory diseases and 20 (interquartile range, 7) cases for cardio-cerebrovascular diseases in Hangzhou City from 2018 to 2020, and the daily mean PM2.5 mass concentration was 29.77 (interquartile range, 21.32) μg/m3. Cumulative exposure to PM2.5 for 5 or 6 d caused the largest effect on the emergency call for respiratory diseases, and an increase in PM2.5 by 10 μg/m3 led to a 1.93% (95%CI: 0.76%-3.11%) rise in the emergency call for respiratory diseases. Cumulative exposure to PM2.5 for 4 d caused the largest effect on the emergency call for cardio-cerebrovascular diseases, and an increase in PM2.5 by 10 μg/m3 led to a 1.88% (95%CI: 0.80%-2.97%) rise in the emergency call for cardio-cerebrovascular diseases. Cumulative exposure to PM2.5 for 7 d caused the largest effect on the emergency call for respiratory diseases among residents aged 60 years and older, and an increase in PM2.5 by 10 μg/m3 led to a 4.37% (95%CI: 2.70%-6.06%) rise in the emergency call for respiratory diseases. Cumulative exposure to PM2.5 for 4 d caused the largest effect on the emergency call for cardio-cerebrovascular diseases among residents aged 60 years and older, and an increase in PM2.5 by 10 μg/m3 led to a 2.44% (95%CI: 0.97%-3.52%) rise in the emergency call for cardio-cerebrovascular diseases. However, exposure to PM2.5 had no marked effects on emergency call for respiratory or cardio-cerebrovascular diseases among residents aged <60 years.@*Conclusions@#Elevated atmospheric PM2.5 mass concentration may lead to an increase in the daily emergency calls for respiratory and cardio-cerebrovascular diseases, notably among residents aged 60 years and older.

ABSTRACT

In recent years, it is noted clinically that the low level of serum uric acid is closely related to the adverse outcomes of cardiovascular and cerebrovascular diseases. Recent studies have shown that low uric acid levels not only boost the incidence of arrhythmia and cardiovascular events, but also increase mortality. It also has adverse effects on the development and prognosis of cerebrovascular diseases, including intracerebral hemorrhage and stroke. This article reviews research advances in the adverse effects of low uric acid on cardiovascular diseases andcerebrovascular diseases.
